Bleeding 2 years after total hysterectomy and oophorectomy

This morning, approximately two years after a total hysterectomy and oophorectomy, I noticed fresh blood (slight, but definite) discharge on my toilet paper. It was from the vaginal area, not the urethra.

Why would this happen?
