Hello, I have been on Roaccutane 20mg tablets for 2 weeks now, and I think it’s working just fine. My dermatologist told me I would be on the drug for 8 months, just to make sure the acne doesn’t return.
However, I would like to have breast implants next year, to correct my sagging ones now. My Question is, can I have the cosmetic surgery while on roaccutane, and if not how long do I have to be off the drug to have the procedure.

You usually have to stop taking it 1 month prior to surgery as Roaccutane also has side effects on your blood coagulation - and that could get very serious during surgery, but most importantly at the healing process (specially the first couple of weeks)
For minor cosmetic surgery - rhinoplasty, ear job, etc. - you should also spend an extra month without taking any of this medication AFTER surgery. You'll have to take a load of pills and combining them altogether with roaccutane just doesn't help your body adjust to it's new phisique.
PROBABLY (I don't know for sure) liposuction and breast implants will require a couple extra weeks as it is a much more invasive surgery - handles much more tissue, that is. However, it is the doctor that will the determine when you can already start taking the medication back again, depending on your physical conditions. Plastic surgeons are very familiar with this kind of situation, though - if your doctor has got enough experience, of course (= that, also, is highly recommended).
I speak from my own personal experience: I was also on Roaccutane and had a nose job recently. Although my dermatologist wasn't very happy on my two-month interruption of the treatment (stopped taking the pills in august, performed surgery in september, restarted in mid october), it was for the best, and when you start taking it again the effects come much faster than on the first weeks! So I guess it's not that big of a deal, really.
However, in my opinion, perhaps you might have to extend those 8 months of yours to about 1 year if you have this surgery, considering you'll have to stay at least 2 months and a half without it.
I'd also definately recommend that you should wait a least 4 months on Roac to stop. Any shorter than that will have the medication's effects practically cancelled if you stop taking it for a long period of time! That is, if you have surgery scheduled for january, it's pretty useless to take roaccutane for just 3 weeks now as acne will come back to its initial level just after a few days...
