Peridontitis cause and treatment

I was diagnosed yesterday with periodontitis - I brush my teeth twice daily how then can I contract this disease? I am a diabetic and it was not controlled for approx 10 years but it is now with insulin injections.

Can I fully recover from this? what treatment is needed?
