Possible cracked ribs - anything else it could be?

My brother was attacked by someone with a spade five days ago. He managed to deflect most of the blows but was caught at least twice on his right side under his arm pit in the rib area. He has bruising and swelling. It is especially painful when coughing, when he feels a knot, like tensed muscles under his ribcage in the front. When massaging you hear a cracking sound. His stomach is also distended and we are a little concerned that it is not just a cracked rib. Could there be internal bleeding - what should we look out for?
