The best way to know if she's orgasmed is to ask her. Most women will tell you if they're finished or not.
Signs of an orgasm include, rapid breathing, tensing of the limbs, clutching of the hands, a flush over the chest, gasping, moaning or whining, and contractions in the vagina or area.
NOT ALL WOMEN WILL EXHIBIT ANY OR ALL OF THESE SIGNS. Some may, some may not.
The key is usually tensing of the whole body, and followed by a relaxing or release.
As for your second question, I can't say that there's any obvious feeling when inside a woman.
But perhaps someone else can share their experience.
