Six Year Old Appears to not be Circumcised

My six year old was circumcised when he was born. However it does not look like he is circumcised. His penis stays hidden and it is hard for him to urinate in the toilet because it does not come out. What does this mean? What should I do? I don't want to inflict pain on him now that he is six. But should he be recircumcised?
