Swelling around left Testical

I have swelling on side of left testical when I am sexually aroused and when I ejaculate. The swelling seems to go down after orgasm has been reached but testicles feel discomfort for a day or so after. Have been to doctor about this but haven't been totally honest with him about when this happens. Can anyone tell me what this is?
