My brother is thyroid cancer patient with suppressive levothyroxine treatment. What can you tell me about 3rd generation of TSH? Does third generation assay allows reliable surveillance of levothyroxine treatment?

A third-generation thyroid stimulating hormone (TSH) assay enables reliable adjustment of levothyroxine dose. It is done through basal TSH measurements in patients with thyroid cancer patients requiring suppressive levothyroxine treatment and it allows effective surveillance of TSH. Basal TSH as measured by the third-generation assay was significantly correlated with values of TRH-stimulated TSH measured by the second-generation assay, also there were close correlations between basal and TRH-stimulated TSH in the third generation assay and between TRH-stimulated TSH in the second and third generation assays. To answer to your question, yes, the third generation assay allows reliable surveillance of levothyroxine treatment using only basal TSH measurements, even in thyroid cancer patients who require a high degree of TSH suppression.
