Unidentified piece of tissue

My wife gave birth to a beautiful baby boy 9 months ago. About one month ago she had her birth control shot. She decided on this method due to the fact that she is breast feeding. Three days ago she started to have her period, but yesterday she was very concerned. She had menstrual pains stronger than she ever had. When she proceeded to go to the bathroom to remove her pad, she encountered a piece of tissue about 2 1/4in. in length, 1/3 in. in with and not that thick. It did not have form nor shape. It resembled a piece of meat left over from what you throw on a grill on a bbq. We are very concerned, she has a doctors appointment a week from now and wanted to get information from the web to see if I could get some answers. Could this be tissue left over from the pregnancy? And if it is why did it take 9 months to come out?
