I am not sure about applying cold or heat but I know that lying on your back should help as this position allows varicocele drainage and relieves discomfort. You may also use some kind of scrotal support or take painkillers like acetaminophen or ibuprofen when the discomfort begins.
Fixing varicocele should eliminate the discomfort all the way.
