chest pain,diahrea and vomitting just about everymorning

My boyfriend jeff will complains that his chest hurts, he wakes up everymorning dry heaving or vomiting. And always has diahrea when he goes to the bathroom (he goes quite often that it causes a rash on his butt) Hes been smoking since he was 12 years old and now is 25. Im gonna guess a pack a day?!? Im just worried about him. He wont go to a doctor unless i basically make an appointment and tell him he has no choice? please help if you can!!
