Severe complications in men are rare but intreated Chlamydia could travel to the epididymis and cause inflammation of this tube that tends to be quite painful. It could also cause swelling and pain around testicles but this would also be due to epididymis infection. Epididymitis may also cause fever and could lead to infertility.
