constant bleeding

I gave birth to my baby girl 4 months ago, and stared back on the combined pill roughly 2 1/2 month ago, since being back on the pill i have had a constant and very heavy vaginal bleeding with clots aswell, i went to the doctor and he suggestd i change to the mini pill, which ive been on for a couple of weeks now and the bleeding is still the same.
i wil be 22 soon and am otherwise in good health.
any clues as to whats causing the bleeding?
although im not in any pain at all, it severly disrupting my life.
