A gallbladder ejection fraction test is done to see how much bile leaves the gallbladder when it contracts and empties. It is done in order to see if you have acalculous biliary pain or "ABP". This is the pain a person experiences as gallstone attacks but the ultrasound showed that no gallstones were present.

Gallstone pain is called biliary colic and it includes upper abdominal or right upper abdominal pain, usually worsening over 15 to 30 minutes and lasting for up to several hours. It could radiate to the right shoulder blade. There is severe nausea and vomiting present as well. When the gallstones are not present, and a perso feels this pain, a doctor may order a gallbladder ejection fraction test.

The theory behind the acalculous biliary pain is that there are that microscopic stones present, too small for an ultrasound but big enough to obstruct the cystic duct or that the pain comes from an abnormality in the gallbladder muscle, which affects the way that it empties.

The test is performed by inserting a radioactive marker into a patient, which is taken up by the liver and gallbladder. Then a hormone called CCK is injected to stimulate the contraction of the gallbladder. The ejection fraction is the amount of the marker that leaves the gallbladder.

Normal ejection fraction of a healthy gallbladder is between 35 and 75 %. If you suffer from “gallstone” pain and have lower ejection fraction results, then you will be diagnosed with ABP.
