foreskin ripped and healed irregulary

my foreskin ripped/abrasions from intercourse a few years back. after a while it started to heal but because i would still constantly have sex it healed the foreskin irregularly causing the mid section of the penis to appear like it has tiny cuts on it. how can i get rid of this so it won't look like it has been wounded?
