heart rate tops out

I was talking to a friend today, he is relatively active he is 41, plays volleyball, and basketball. He was telling me of a drill he used to do in younger days as he jumps up the stairs of a football stadium he eventually gets exhausted but his heart rate doesn't get above 135. What could cause this? His resting heart is pretty low as well in the high fourties. Thx. Steve
