loose foreskin but already circumcized at birth

my friend has had an issue for almost 30 years now. he has loose skin on the shaft of his penis and has to hold it down at the end of it while having sex. Remember, he was circumcized at birth. He says that sometimes it hurts him to have sex because of the excess skin. he also thinks its getting worsed and more stretched out the older he gets. he is 50 years old now. He had gone to a few doctors in the past and then all tell him notbing is wrong. well, if that is true why does it hurt him?

Any suggestions on what kind of doctor other than urologist, he might go too? anyone else have this issue?

Sounds like he only had a part circumsision.

I would suggest he go and get a full one if that is the case
