It may not be pain at all. Children can’t express what they are really feeling, so they may refer to any changes as pain. He may feel spasms in the bladder or bowels if he is not emptying fully and may describe the sensation as pain.
Have you reported erection pain to the doctor who examined him? Has he considered an ultrasound to have a look at the genital organs?
Children who are not potty trained rarely suffer from UTIs and urinalyses has already ruled out infections, so maybe an ultrasound could reveal an anatomical problem.
You may ask for a second opinion and make sure you report to the doc all the symptoms you notice and when your child complains the most.
