on and off testicular pain

This past week my husband has mentioned his left testical hurting. I've asked him a few details about it and the pain comes and goes, he compares the pain to a bad backache. The pain lasts for maybe 30 minutes at a time and only comes every other day. He doesn't seem to have any pain urinating or during sex. His testical swells while it is sore but otherwise looks normal, not always swollen or red. Does testical torsion come in waves or is it more likely a bacterial infection? I'm concerned for his fertility and general health. He's not wanting to call the Doc and doesn't want me getting all worked up over it either. I don't want him to loose a testical because he didn't want to go to the Doc. Any advice or ideas on what this may be?
