painful erection

Hi, my son for the last year and a half has been having painful erection after he uses the bathroom when it started but know comes at any time. We did test (sickcel and ultrasound)at the hospital, the result showed nothing. do you have any sugestion to what could be causing this type of reaction. He is 8years old, and the problem comes and goes for both long and short periods.
