pains in anus testicles

i have pain in around anus(random spasms), pain (random)spasms in testicles, blood in toilet tissue after stool, pain urinating, stool is harder-smaller parts coming through. bleeding has been small but has been going on for several years, other symptoms have been going on and increasing in frequency over last week, pain can be be quite disabling for a few seconds, unsure what it could be, please help!!!!
