very irregular periods recent spotting

I am 22yrs old i have had irregular periods since i was 15 but i began my cycles at 12. My spouse and I have been hoping to become pregnant for the past 5 yrs now but so far nothing. My most recent period was in aug and now 3 mo. later im experiencing some spotting for about 2 or 3 days the past 3 weeks along with mild cramping, fatigue and nausea. What could the problem be? Im afraid of taking a pregnancy test cause each time in the past i've only been disappointed. Are there any other explainations for this?
