Can a doctor tell if it was an abotion or a D&C?

Would your doctor tell if it was an abortion or D&C? My cousin is so worried about telling her doctor that she wants to say she had a miscarriage and a D&C. Could a doctor know? The two are the same surgery so thats why she wants to say this. I do not know so thats why I am asking this...
