heartbeat pounding in the same rhythm as my ear sound

Chest pain, I went to gym yesterday and did BACK exercise, after an hour I felt pinch in my chest.

Whenever I breath I feel pain in my epiglottis and chest. I am not even able to take deeps breath so what should I do to get rid of it. Year ago, I had same problem and at that time also, I was doing GYM. Actually I do not want to quit GYM. So Sir please help me. Thanks.
