Yellow scabby nipple on each breast

For as long as I can remember, my nipples have been yellow and scabby, and it doesn't look right at all. I'm 15 and I'm really worried something's wrong. Bits of them easily come off, but they grow back, yellow too. Could you tell me what's wrong or what I could do? Please.
