cataract surgery with astygmatism - is lens covered by Medicare

My opthalmologist told me that since I have astygmatism the insurance does not cover the entire cost. I would have to pay $700 over what Medicare would pay. Is this the general policy or is it the particular medical practice that adds that cost?
