My foot is numb

My right foot is numb (that includes all toes) and has been for over a year. It hurts to touch in places but its just a constant numbness - I mean every day for over a year its been numb! Facts- Im a smoker Im overweight - very much so. I have high BP I have arthritis Any thoughts?
