upper abs spasms due to excercise

I have been working out with a trainer for over a year. Sometimes when we do ab excercises I get severe spasms in my upper abs. I was diagnosed with a hiatal hernia over 5 years ago. Could this be the cause? What are some good ab excercises to do that I could handle?

I have chest pains midline and in the left breast. I do have a hiatal hernia. But I donot have trouble with acid reflux. The pains were just now and then but now I have them everyday. The pain is sharp and last for several minutes.
