My testicle spins around freely and ascends in a forwards protruding way

My left testicle spins around freely right and left, upwards and downwards 360 degrees. When i get erect and my scrotum tightens , it ascends and ends up being protruded forwards not aligned vertically with the right testicle. The ball in my scrotum becomes painful like the other balls when you squeeze them

 The ball in my scrotum becomes painful like the other balls when you squeeze it.it is also growing in size and the other ball to close to it seems like its dissolving if i should say. The GP i consulted did some blood tests for the cancers and ruled them out what could be the problem
