im 15 and i was wondering if it was normal that sometimes my nipples and areoles puff out above my breast and are smooth instead of the normal kinda wrinkly way they're supposed to be?

The skin on both of my breasts is extremely dry and flaky always. I have a lot of trouble with breakouts too. I have tried changes with my soap and detergent and it's still awful. Is this normal? What could it be and what should I do?
