discoloration on nipple, very painful and extreme breast tenderness

I am 45 and have a brown discolouration on my right nipple but no discharge, the nipple is very sore, my breast is extremely sore and it is very painful to wear a bra or even just a t'shirt without pain. any ideas?

What are these purple spots coming up on my boob;

Nipple Color Changed
