What would be the easiest way for me to tapper down from where I am

I am very concerned and want advice on tapering. I have had 3 back surgeries and they are saying I need a fourth thats not why I am here. Truth is its just another drug with horrid wd symptoms that just dont seem to be fading and im in the eve of the eight night. I started subutex in May tapered down every month just as Dr ordered until 1 Mg per day...still wd is the worst..I have 5 kids here to take care of a husband to take care of...
