Pain in right testicle which goes on and off for the last 20 years

I have testicular pain off and on for 20+ years. It comes on suddenly in right only and feels as if it is drawn up inside but it is not. Lots of pressure as if I have been hit in the testicle area "racked" Is this common, what may be the cause?
