Fentanyal patches up to 100 irritating the skin like a burn stings slightly

My grandad has been on fentanyl patches for roughly 5 years in the last week hes dose has been raised to 100mg. The patch has caused a red rash that slightly resembles a burn and is causing slight pain. Is this normal and what has all of a sudden caused it after so long?
