dilaudid and fentanyl

My husband is on 4mg tables of Dilaudid every two hrs and 125 mcg of fentanyl. His back pain appears to be getting worse and they keep raising his fentanyl. Could one of these two drugs actually be making the back pain worse? Anyone have any experience?
