Fingertips severe pain in all digits

Can acrylic nails (worn 20 years) cause severe pain in all fingers and both thumbs with tingling, sensitivity? What would be the diagnosis? No dr. knows. Last hope is morphine. Patient is 86 yrs old female. What type of dr. should she see?

Painful, popping elbow that locks
