My right testicle has enlarged.

for quite awhile, i have had itchy pimples on the backside of a red inflamed scrotum. they are also between my scrotum and my legs. i have thought it was just a heat rash, and i have had no complaints from my sexual partners (heterosexual) in the past or present (monogamous). can you help?
