Diabetic with neuropathy in both feet on 32 mg dilaudid im a day!! Can anyone help?

I am a 54 year old male with diabetic neuropathy, Recently I have been getting cramps every four hours that extend from my toes to my calves. This severe pain stays about 10 minutes. Is there anyone else out there on this much dilaudid with this problem what have you done to prevent the cramps....
