can't urinate

i HAVE NO PAIN JUST CAN'T URINATE. I SOMETIMES ONLY PEE ONCE A DAY AND IT'S HARD TO DO IT. IT TAKES ME A LONG TIME TO PEE.

When i uinate it sometime itches so much that I have sometimes gotten the idea of inserting a straw in my urethra to ease the itch and after ejaculating I get this excrutiating pain that makes me want to amputate my penis. Hello, guys. I have some problems with my bladder, I am not sure which one. In any case, I have frequent UTIs. My urologist recommends me to treat it surgically. I am not sure whether to do this. I would like to hear your opinion. This will mean a lot to me. Thank you in advance.
