Continuous Bleeding while pooping

Hi, I just want to ask. Actually this is not the first time. Every time I will poop their is a bleeding. I just don't know where it came from. Sometimes it looks like a poop I mean every time I will poop I thought its a poo then when I check the bowl it's a blood.

I had an uncomplicated appendectomy three weeks ago.  It had not perforated.  I'm still experiencing pain in my right shoulder and below my right ribs when I inhale.  I thought the gas should have dissipated by now.
