irregular menstruation

this is the second time im having an irregular menstrual flow, and i don't know what could be the cause. the first irregular menstral flow started 21 days into the cycle instead of 28 days. Now this second one is 15 days into the cycle. Please what could be the cause?

Okay..... I was 12 days late for my period then when I finally got it my period it only lasted for a day and a half, and now I have this really dark brown discharge that comes out from time time and when it does come out it is really thick..... What do ya'll think this could be??????
