
I am posting this to hopefully help others who may be experiencing incontinence following HOLEP. For 7 years after my successful HOLEP in 2017 (Dr Das in Philly) I experienced ongoing mild to moderate stress incontinence. Due to the length of my prostate, Dr Das had to do a lot of work to complete the procedure and unfortunately it left me with problematic incontinence. Coughing, sneezing, lifting, running, all caused varying degrees of incontinence. I usually went thru 1 - 3 CVS light-duty shields a day. I work as a produce stocker at Wegmans which requires a fair amount of daily lifting in the 30 to 50 lb range. Needless to say, this made life as work difficult leaving me quite miserable.
Now to cut to the chase: I was treated for kidney stones in the winter of 2024, and after discussion with my urologist, explaining my ongoing incontinence issues and the misery it left me in, he advised that there is a procedure that could be very effective for me: AdVance XP Male Sling System (Boston Scientific). He referred me to a urological surgeon with extensive experience with this procedure, Dr Edmond Paquette in Fairfax VA. Dr Paquette advised me that he had never done the procedure on a HOLEP patent, but after we reviewed a paper from Baylor University that showed success with this system on 3 post-HOLEP patients, we agreed to give it a try.
To make a long story short, after out-patient surgery on April 4th, and 2 days with a catheter to aid recovery, I have been 100% completely dry ever since! This procedure has been a God-send! I have my normal life back! No more shields, no more avoiding a full bladder for hours a day. I hydrate and urinate normally for the first time in over 7 years. Recovery involved 2 weeks of mainly bed and light walking followed by 4 more weeks of no squatting or lifting more than 25 lbs, but after completing this recovery process, I have returned to 100% normal work duty with no leaking at all. I have returned to my normal active life, and I owe it all to Dr Paquette and this procedure.
I post this to hopefully help anyone who post-HOLEP, may be suffering from incontinence. I obviously can't guarantee success for everyong, but it could be worth it for any other sufferers to consult with their urologist about this procedure.
