I'm new to this. How do I reply to my previous post to let people know about my progress and answers that I have found regarding my condition?

Its Easy -- click the relevant button ("Reply" "Reply to this post") on either the discussion board or topic screens. The facilities available to you are listed at the bottom of the discussion board and topic screens (the You can post new topics, You can vote in polls, etc.)

Also concerning tracking of reply's made to your topics/posts you have option below the input field on each post " Would you like to receive an email when a reply is posted?" which if selected option "Yes", will send you automatic email notifications with updates to topics you are participating. If you just want to track reply's on topics without participation, you can use "Subscribe" button which will send update notifications regarding that topic.
