Well, the shot can be painful (but not that much) if it is injected into a joint. But if professional that is giving injection is well skilled injection should not be painful. Besides cortisone injection is often performed with very small needle that is mostly not painful. But, if larger needle is used (to remove the fluid before injecting cortisone), that might be more painful. In such cases, doctors usually use numbing medication to provide temporary relief. This numbing medication is injected together with cortisone to provide relief of the affected area.
