I'm sorry to hear you're experiencing discomfort. There are several potential causes for the symptoms you've described. Some possible explanations include:

  1. Gastroesophageal Reflux Disease (GERD): This is a condition where stomach acid frequently flows back into the esophagus, causing irritation. It can result in pain or discomfort in the upper abdominal region or behind the breastbone.

  2. Gastritis: Inflammation of the stomach lining can cause pain in the upper abdomen.

  3. Gallstones or Gallbladder Disease: Stones in the gallbladder can cause pain in the right upper abdomen, often just below the ribs.

  4. Irritable Bowel Syndrome (IBS): IBS can cause a variety of symptoms, including abdominal pain, bloating, and changes in bowel habits. It can also sometimes cause rectal discomfort or a feeling of incomplete evacuation.

  5. Hemorrhoids: These are swollen veins in the rectum or anus that can cause pain, itching, and sometimes bleeding. They could potentially cause the needle-like sensation you described.

  6. Anal Fissure: A small tear in the lining of the anus can cause pain, especially during bowel movements.

  7. Proctalgia Fugax: This is a condition where one experiences sudden, sharp pain in the rectum that lasts only a few seconds or minutes, then disappears. The cause is not well-understood, but it is believed to be related to muscle spasms.

  8. Other Gastrointestinal Conditions: Ulcers, infections, inflammation, and other conditions can cause pain in the stomach or gastrointestinal tract.

  9. Musculoskeletal Pain: Sometimes, pain in the rib or diaphragm area could be related to muscle strain or other musculoskeletal issues.

  10. Other Causes: There are many other potential causes of abdominal pain and rectal sensations, ranging from infections to more severe conditions.

Given the range of possible causes, it's crucial to consult with a doctor  who can conduct a thorough examination, ask specific questions, and order appropriate tests if necessary. They can provide an accurate diagnosis and guide you on the next steps.
