You may be describing rectal prolapse. If so she may even require surgery to treat.
When you pulled out fast you created a suction. It tore the tissues.
Don't be embarrassed, these things can happen. They are doctors and have seen everything.

If she's in a lot of pain and it appears that the anus is inside out then she needs medical attention. It's the inside out appearance that concerns me. This could indicate that the connective tissue that holds her rectum in place has torn. She can have serious problems with bowel movements, possibly long term.
Is she over 18? How about a clinic or something? Just for an evaluation to determine if she needs treatment.
I can understand you're both scared. Hospitals will work with you if someone requires treatment, they can't refuse.
