Hello Everyone:

I am 16 days post surgery and I am doing very well. (for the most part) I had three external/internal hemorrhoids removed. Below, I have a few strategies I'd like to share that have worked for me. I also have some questions regarding diet and exercize.

FYI: If you have a job that does not allow you to go the the bathroom at your convenience, I would plan on 3 weeks off work. I am a substitute teacher. I went back into the classroom for one day last week, Thursday, and it was too much. I wore a depend (just in case) and I had a super day with the kids, teaching and energy wise. I subbed at my kids little Catholic school and there were only 16 kidsin the classroom. I had to leave the children unattended twice to rush to the bathroom. I didn't feel that was an acceptable position to put myself or the kids in. I normally sub for the public schools and the private school was a trial run.

To the folks who are having spasams (I am so terribly sorry, what hell). I have had a few 30 sec spasams, I think. I have dependedly HEAVILY on my WATERPICK DETACHABLE SHOWER HEAD!!!! My husband even ran out and purhased a new one for $20. I use it approximtley 3 times per day and more as needed. The pulsating stream cleans, massages and helps greatly with itching. I would think that the massage of the muscles and tissues in that area would help with spasams.

I wear a depend on occasional outings. I had sweat running down my cheeks and had to laugh!!! It took me awhile to figure out what was going on. If you think about it, it makes sense. I've found that if put my COTTON underware on first and then the depend, that I am much more comfortable.

To the person who posted the warnings regarding fisures, God Bless! My Dr. did not warn me about spasams or the risk that they put you at for fisures.

I am eating brown bread with 5-6 grams of fiber per slice, lots of water, prune juice, dried and stewed prunes. I take Milk of Magnesia every evening before I go to bed and I take 4-6 stool softners every morning. I was told that I could eat what I wanted and now I'm not so sure.

My Dr. perscribed Valerate cream to help with the excessive itching. It has worked great for long periods of time. It's definatley better than hydrocrodisone 2.5%. I also purchased a lubricant laxative KONDREMUL. I got it at the Dr.'s office. It's main base is mineral oil and it does not promote diarrrhea, but is supposed to assist things sliding down the ramp. I just started using this, so the jury is still out.

QUESTIONS:

Can I really eat normally and not be constipated? I am very aware of fiber and fat/spice content. I am struggling with constipation and straining. What kind of diet are you folks on? There was a previous posting suggesting eggs, soups etc...

When can I do Pilates/yoga? I signed up for a class and have missed 3 sessions. I would really lilke to go Tuesday and I am concerned. What do you all do for exercize?

What is the status regarding laxatives? If the laxative is stimulate free, like Milk of Magnesia, can you take it FOREVER? At this point, I can not imagine not taking an aid. I take 4-6 stool softners and am scared to stop, but worried about long term use. I also take Milk of Magnesia every night before I go to bed and I am using the Kondremul lubricant laxative as well.

I really enjoy reading your notes and comments. You are so right on! The people on this website are the only ones I can truly share my triumphs and tribulations with. It's just plain gross and way to personal to discuss....and at the same time, I feel a bond with you all and I cling to your words of wisdom and sense of humor.

Your idea to offer the diary support to others and incorporate a specialized treatment/spa resort sounds heavenly.

To the person worried about losing your job, how are things going? I think of you often and wish you all of the best.

Before my surgery 21 days ago, I read every post in this log and frankly, got so worried that I considered cancelling the surgery! I'm so thankful that I decided to move forward with it.

I suffered with daily prolapsed hemorrhoids (internal hemorrhoids that fall outside of the rectum) for more than 10 years. In the last year before my surgery, I had constant bleeding, seeping from the rectum, and pain often to the point of not being able to walk or do daily routine activities. I altered my life style so that I could live with the hemorrhoids, i.e. I wore only black pants, panty liners everyday, didn't bicyclye or walk any distance, laid on the couch as much as possible and even altered the way that I got up and down from a sitting position in the hopes that it would not hurt as much.

On January 5th, I had a radial hemorrhoidectomy; radial being all three arteries were effected. The surgeon used numbing medicine below the waiste during the surgery and the effects lasted for about 10 hours. Consequently, I did not feel much of anything for several hours after the surgery and did not need pain medication until about 9 hours afterwards. I had the surgery at 7:30 am on a Friday and was held overnight for monitoring. I got home around 3:00 pm on Saturday.

During the first week, I had two bad episodes. One was in the hospital when the medication (morphine) was ineffective; fortunately, this lasted only for a couple hours until they switched me to another narcotic that was more effective. The second episode was on day 5. I had not had a BM even though I was taking Metamucil, Milk of Magnesia, Delcolax, and a senna laxative. After suffering through the day, I was finally able to reach the surgeon who suggested an enema provided that I thought I could handle the pain. At this point and after suffering the constipation, I knew that the enema would solve the problem and I went for it. And then I screamed, cried, and wanted to vomit! The pain was almost unbearable during the BM and I think I sat in a hot bath for about two hours afterward, but by bedtime, I was feeling better.

Since day 5 and the enema episode, I have progressively gotten better. I use a ring when I sit down but am able to sit for hours at a time. Three weeks post surgery, I am still using Motrin, stool softeners, and Metamucil. However, I have daily BMs and provided that I watch the type of foods I eat, they have been fairly painless for the past week or so.

I feel like my surgery was a success and am very thankful that I feel comfortable enough with my surgeon to talk very openly with him. I certainly think that because of the severity of the pain I was experiencing prior to the surgery, I was able to cope with the post-op pain fairly well.

Here's some of the things that I did to help me after the surgery (things that I hope help you also):

1) Lortab - I took these as needed and went through 30 500-mg pills the first week. After the first week I quit using Lortab.;

2) Motrin - I took ibuprofen at the drop of a hat and without regard to the suggested dosages. Consequently, I went through a bottle of 100 within the first 16 days. In the third week, I now take about 3 - 6 a day.;

3) HOT baths - The first week I sat in a hot bath as often and for as long as possible. I would guess maybe three hours a day. I'm down to just my normal once a day shower now.

4) Rest - I am still sleeping as often as my body allows me. I will have to give this practice up when I return to work next week.

5) 4 weeks off from work - I'm thankful that I was able to take 4 weeks off from work after the surgery. Since I just completed week 3, I still have another week to go and am pretty confident that when it is time to return, I will be ready. Unfortunately, not everyone has the opportunity to do this but I would suggest taking 4 weeks if at all possible. It relieves the stress of having to return to work too soon and gives your body time to heal.

6) Metamucil - Twice a day with a 8 oz of liquid for the rest of my life! I use apple or grape juice and it doesn't taste so bad!

7) Enema for severe constipation

8) Delcolax - Twice daily. I asked my surgeon about the long-term effects of using a stool softener and he said that provided there are no hidden problems, a stool softener can be used long-term. Since I was checked out prior to the surgery, we know there are no hidden problems so I plan to continue taking this medicine indefinitely.

9) Daily vitamin

10) Lots of fresh fruits and veggies and high fiber cereals. I am avoiding ALL remotely spicey food, including things I don't usually think of as being spicey, like spaghetti sauce. I have started to re-introduce meats to my diet this week but find that it makes the BMs a little (not nearly unbearable) uncomfortable. Since this is the only thing that is causing discomfort now and can result in an all day discomfort, I am back to avoiding meats.

11) Benadryl cream for the itching!!!! It does wonders!

My stitches are gone now and everything is thankfully, pulling back up inside. I've had very little post-op bleeding. I can't believe that I can actually move around without worrying about a sudden surge of hemorrhoid pain! And no more pantyliners!

I hope that your surgery and recovery goes as well as mine has. For those of us that had years of pre-op pain, bleeding, and seeping, I think it's the only solution and a decision that you will be happy you made!

Well I had it Dec 22, 2006, lots of internal, rectrum repair, 21 years ago had a very large child. Also had prolapsed bowel surgery in Sept. 2006 Day one I thought it was a breez..no pain, felt great, sent home that afternoon, very drugged. Well the next day the hospital called just to check and I said it was starting to hurt, take your pain pills now, do not wait until you can not stand it, it takes at least 1/2 hour to take affect.

Where they right...this has been the worse surgery that I have ever had and I have had 3 and stayed in for a week for two of them.

I did not have a bowel movement for a week, (have IBS) and the pain medication bounds you up. I was taking laxetives, mineral oil, you name it I was taking it, Also Aleve for swelling. And sitting in the tub for hours. My sons had to call my husband home, I almost passed out, lots of bleeding.

I can not believe you do not stay in the hospital for this, my father in-law did years ago. New years was a not much fun time to go again....ill, could no sit down.

Off for 4 weeks, 4 weeks of bleeding, wich I was told was normal. After 3 weeks doctor, took pain bill before going, he had to cut 3 stiches, I thought I was going to come off the table. He said I was healing well, take off another week, come back in 6 weeks for a check up.

I know it will be worth it in the end, everyone who has this needs to take good care of themself.

hello, it depends how big the surger is. i just had one last week about 5 days ago and it hurt so badly. im still at home recovery. my dr said to be comftable, it may take up to 6 wks. it really depends. it can get better before 6 wks, but i dont think it will be 100%. im suffering from such horrible pain. i cant even move from side to side, it hurts when u do. it also hurts when u try to stand up and sit down. i do agree on the sitz bathtub. that really work. it kind of ease the pain for a bit. i suggest to do it right before u go to bed so u can actually get a better night sleep. i was suffer from not able to void completely. i did not sleep for 4 nights. i mean as a good night sleep. i was up every hr. now its the pain on the rectum. it itch so much and it burns. also when your buttcheeks twitch, (squeze together) u will feel the worse pain ever. i dont know if this is something that everyone is going thru too, or just me. my dr said because the muscle is tight and has not ralax yet. im like, oh boy. this will keep me up for a very long time. well i dont mean to scare ya'll, but thats what i have experience so far.

I feel so bad for all of you! It's been just over three weeks since I had my triple / radial surgery and I am doing very well. I wish all of you were!

3907huron, I think having to wear Depends would be terrible! I am assuming you are using these because you have little control over your bowels. Correct? I'm wondering if perhaps the amount of laxatives and stool softeners you are using are too much. I did have problems with control through I'd say the second week, however, everything seems to be working fine now (three weeks post-op). I take no more than two stool softeners and a dose of Metamucil once a day within an hour of going to bed. Although the stool is firm, it is not difficult to pass or cause much discomfort. I then take a hot bath and am ready to go for the rest of the day with a few ibuprofen. I don't think you can eat anything .. especially spices, from experience, I know that causes alot of burning! I am eating a high fiber diet and am avoiding ALL spices. I also watch how much meat I eat. Per my surgeon, provided that you know there is not a "hidden" problem that could be causing constipation or abnormal bowel movements, long-term use of stool softeners is fine and daily use of Metamucil, which is really a fiber laxative, is encouraged. 3907huron, I encourage you to write out your questions and call your surgeon's office. I know that my surgeon has been very helpful and patient with all of my questions. Hopefully, yours will be also. You should not have to be wearing Depends!

Hi all,

I went to the specialist today for my 2 week post op checkup. He asked how I was feeling and I enquired whether he had misplaced the keys to his Lexus or maybe his left shoe, if he had i had a pretty good idea of where they might be. He thought this was hilarious (mean SOB).

really I guess I should not complain. It's 2 weeks and one day since the operation and given that it was a very severe case I could be in a lot worse shape this far into the recovery. I am uncomfortable but not in real agony anymore. I have BM's every day at least once and although they are uncomfortable they are also not incredibly painful.

What is really painful is the period immediately after a BM. As I have mentioned before there is always a little left behind (excuse the pun) and until my body expells this I do have quite severe pain. this can take from 1 hour to 4 hours after a BM. It is such a tiny amount that it is amazing that it can cause this much discomfort but it must put pressure on the internal scar or the rectum ( or his Lexus keys). what does relieve it is when what I have been calling my bodies natural repair ooze takes over and ejects this small amount of residual poo.

When I explained this to the speciaist he said that because the operation and sheer amount of stitching inside there was probably a tender scar tissue being affected. He took a look and the problem is a little more simple. I have developed an internal infection and the pain is related to that (so much for the bodies natural healing ooze, it's really the product of the infection....yuk). So I now have 2 types of antibiotics as well as the other multitude of pills and medicines I take every day.

Apart from this little setback I am feeling ok. As I have said before I cant imagine how anyone could function in a workplalce while recovering from this operation. I have cancelled all shows and put all business on the back burner until this is well behind me (hahahah...another pun).

I will post another update in a few days once the antibiotics kick in. To be honest I am a little worried of the effect they might have. If they stop this ooze altogether then will I have this lingering pain all the time....hhhmmmmm.

For Lecia and anyone else suffering from anal fissures,

This is what I know about fissures: (Not in any particular order)

1) see a colorectal doctor (not a gastro or general surgeon)
2) your doctor does not need to put his finger in your rear to examine but should be able to assess by simply spreading your butt cheeks apart
3) if doctor insists on attempting to put his/her finger in, find a different doctor
4) chronic fissures (ones that keep returning or don't heal after a short period of time) need treatment
5) if topical treatment is obtained early, surgery can usually be avoided,
6) topical treatment of fissures would include nitroglycerin (NTG) .2% (can cause headaches), diltiazem cream (2%), nifedipine and other medications
7) the creams can work, but if your injury is extreme then you may need more drastic measures
8) soaking in a warm bath helps (do not use hot water)
9) a sitz bath (can be purchased for about $10) is nice for relaxing and cleaning after a bm
10) suppositories can increase the pain (and sometimes can thin the tissue), regardless of type (anusol, prep h, glycerine, etc.)
11) overcoming the fear of having a bm is hard to do because you never know ahead of time what the outcome may be
12) watch your diet - add fiber and avoid the following foods: beef, cheese, spicy foods, nuts, popcorn (also less in is less out)
13) drink LOTS of water (I drink 90-120 ounces a day)
14) bulk and soften stools (I use Metamucil powder and Colace before breakfast and before dinner)
15) lubricate the area before you have a bm (I use KY Jelly) I have heard BAD things about using Vaseline (a fellow fissure-sufferer told me that gasoline is a similar word for a reason)
16) get treatment asap, if your fissure hasn't cleared up in a reasonable period of time it may get worse, if it gets worse be prepared to suffer for a long time unless you opt for LIS surgery or other alternatives
17) if you are having trouble getting help fast, keep trying, there is no reason to suffer any longer than necessary - also seek out at least two opinions before having ANY surgery
18) the more intense pain hours after a bm are caused by muscle spasms in your internal sphincter
19) there are alternate therapies and treatments that have worked for some people like Botox, acupuncture, dilation, or hyperbaric oxygen therapy (HBOT) - a lot of these things are NOT covered by insurance
20) LIS surgery (sphincterotomy) works well for most people who get it but it does carry the risk (1% or so) of fecal incontinence and it does permanently cut your internal sphincter
21) I read online at a colorectal health site that it takes 3-4 months for the tissue to regain 40% of its original strength and 2-3 years for the tissue to be back to 80% of its original strength. Have not yet heard if it will ever be back to 100%.
22) take your vitamins. I also take a supplement called L-Glutamine which is supposed to help strengthen the tissue down there.

I know a lot of people say the sphincterotomy is an easy, quick way to heal your fissure but I still don't like the risks that are associated with it. My doctor told me that it appears that I am healing slowly so I will continue to use conservative measures at this point. It is a long process to heal a fissure. Keep things soft and self-moving. Good luck!!!

After reading some of the other experiences, I'm feeling better about my choice of surgeons.



I had a triple hemorrhoidectomy just over a week ago (1/24), using Dr. Who's sonic screwdriver -- er -- harmonic scalpel ("harmonic shears", technically) -- a J&J ultrasonic gizmo that cuts and fuses at the same time. It was done out-patient: in at 9am, home by 1pm. No trouble urniating right after the surgery, and they gave me two percocets because the local was supposed to wear off quickly.



Well, later that night, the local *did* wear off. I called the doctor's service, and got a call back. Turns out he was right: I didn't even know I was in spasm, but that's where a lot of the pain came from. He had prescribed Valium (diazepam) for spams "as needed", but I was trying to stay away from them.



I tried one dose of the chemical laxative he prescribed, but the problem is the opposite: I've been going four to six times a day! Each time feels like I'm passing sulfuric acid. The pain has been preventing complete evacuation; thus the repeated trips.



I haven't seen anyone else mention it, so I'll pass along something my doctor suggested that helps a lot: He suggested aloe wipes. Being an old bachelor, I had trouble finding them in the store -- Ayup: baby wipes. They really take a lot of the sting out of a BM. Another aid is a squeeze bottle I've been keeping in the bath -- a homebrew bidet. Between the two, I don't have to jump into the bath immediately. (I also got a wand shower head I installed the day before the surgery on the doctor's advice.)



For those considering the surgery: there will be times afterwards that you think it may have been a mistake. But keep a log of your pain (1-10), and you'll see progress: slow, but progress.



For me, there wasn't much option: I had already tried the Diosmin treatment (Italian study '94) to reduce bleeding, but mine were too far along for that: classes 2,3,and 4 (doctor talk for one internal bleeder, one permanently prolapsed, and one that went both ways). The amount of bleeding was getting alarming. The amount of bleeding since the surgery hasn't added up to what I was dropping from the hemorrhoids in one episode.



I was a bit concerned that I still have trace bleeding from above the spincter, but apparently that's not uncommon after only a week.



The biggest issues I have now are that I don't yet have any "push" for that last bit of a BM, so I have to keep wiping the oozage (and wearing "Depends" -- the worst part about this kind of surgery is the insults added to injury. :))



And I think I'm getting diaper rash from all the baby wipes. ;)



(Actually, the doctor's instruction mentioned using Desitin, A&D ointment, or cocoa butter around the area. I'm learning to pay attention to all the instructions.)



My doctor predicted that recovery would be a week off work (and 4-6 weeks total). That looks to be optimistic.



As I was told, the skin tags are left to err on the side of caution -- if they snip tight enough to make you drum tight, there's a risk of incontinence -- the spincter might not close completely -- and wearing Depends just for a couple of weeks is bad enough.



I was trying to stay off the meds (I switched to plain tylenol from percocet), but I think after reading the connection between spasms and fissures, I might go take a half a valium.

I posted this previously regarding spasms. My doctor recomended capsules available here in australia called MINTEC. They are a natural remedy for IBS and they work a treat for spasms. they contain a small amount of peppermint oil incased in a capsule that does not disolve until it reaches the bowel apparently.

I guess that an alternate health practitioner might have something similar to these in the US

Hello Everyone:

I can't thank you all enough for your honesty and wisdom. I am doing all of the things that have worked well for most of you. I'm going back to work on Monday and I can't wait. It's been 3 weeks.

Getting Nervous in Ohio: Thank you so much for all of your information. I did ask my Dr. those questions at my follow up appointment. I also re-read the instruction sheet that I was sent home with. I am not wearing depends anymore and have got the whole laxative, water, fiber thing down. (So far, so good)

Graham: I will keep you in my thoughts and prayers. That whole infection thing has got to be the pits.

Fissure posting: Thanks a million. It's all of the detailed information and knowledge that makes the difference. I will keep your posting for future reference, and hopefully never have to implement those strategies.

I feel like I'm ready to fly the coop! I'm in the home stretch now and am praying that I don't have a set back or fissure. Thank you all again for your time, effort and concern. You've been a true blessing and comfort to me.

Hello all again,
Well here i am again reporting on recovery,it has been 13 weeks now since operation as i said before i had triple hemorrhoidectomy and a Fissure i had the usual small spasms when haveing BMS,also pain varied over the weeks from 1-9 got better as weeks went on now i am 99% free from pain i no longer bleed since operation and i am regulary passing soft BMS every other day which is great but it seems now though i do have 2 more outer hemorroids but these are not a hinderence and i hardly notice they are there theses came about after operation due to the spasms and straing when i had to go to toilet during post operation but anyways the problem i do have now is i have not had a hard BM since before operation is this normal!! also i have pencil like BMS but i am feeling okay no pain anywhere just soft BMS when i do have a BM it lasts for 5 minutes and thats it.I seemto have 2 days worth in the 5 minute duration and then thats me for another 2 days is this normal i do keep fit by running now am i still recovering does anyone else have this problem!!. But as others have said prior all does get better slowly first month or so is painful but believe you me now i feel 10 times better since operation just worried now about BMS can anyone advise.

Hi all,

I can only echo the post above (huron). It is now 3 weeks and one day since my operation and I have just returned home from a checkup with the doctor. He is pleased with my recovery and so am I.

We went out to dinner last night and although I: had to take my trusty rubber ring to sit on I had a good night and sat for over 4 hours.

The last 3 days have been the turning point. Up until then I was in "sick mode" I layed in bed most of the time feeling very sorry for myself. My boredom threshold kicked in and I got up and started to do some work. From that time on I have improved remarkably.

The big test will be this Thursday and Friday nights as I have 2 shows booked so we will see whether this is mind over matter or not.

I am having regular and somewhat uncomfortable but not painful BM's and the painful after period is getting shorter every time.

I hope you are all on the improve and thanks so much for the support and encouragement.

The NIH here still lists the use of peppermint oil for colonic spasm as "more research needed".



Peppermint oil is available as "a herbal supplement", but there are warnings about total menthol (as part of peppermint) intake, since so many other US products (mouthwash, toothpaste, cigarettes, cough drops, etc.) also contain peppermint and menthol.

Thats interesting, thanks for the information. I used these capsules 3 or 4 times and they worked for me but to be honest I did not research it at all. At the time I was in so much pain I pretty much would have taken anything to try and get some relief.

Hi,
I had this surgery and believe me know one can tell you how painful this surgery is. I can remember praying to God to please let me have a bowel movement without breaking out into sweat due to the intense pain. I ended up needing a much longer recovery time than they suggest. However, now I am happy that I had it because the hemorrhoids will never bother me again. Simply put, just prepair youself, this surgery is no joke!!!!!!
