Follow up with your doctor.

You have been diagnosed with Pulmonary Tuberculosis in both lungs.  It is a SEVERE infection, from the upper and midlung areas being affected.  It normally starts in the apices, tops, of the lungs.

The soft tissues in your right lung need to be identified.  They could indicate scarring but may also be something else.

The pleural reaction/effusion, basically there are tissues, the pleura, that separate the lungs from the chest wall.  These tissues are inflamed or you have increased fluid.  It may cause pain.

They are requesting a CT scan for a better image of what's going on.

Your doctor should be able to provide specific answers.  Without seeing the x-ray I can't go into more than "general" information.
