When im with a girl, as in kissing for a while, my testicles often become very hot and painful, which can last for some time. I dont have any problems masterbating however.

I reckon that this is because you get excited and a lot of blood runs down your penis and testicles as if you were going to have sex. This blood should leave the testicles upon ejaculation and orgasm but since you don’t orgasm, the blood stays, the pressure stays and you feel the ache. Haven’t you ever heard of the blue balls?
When you masturbate, you orgasm and get relief and some time after the blood leaves the testicles and there is no more pressure, so you feel relieved and don’t experience any pain or discomfort.

hey cool other people get this too!

it used to happen to me especially badly when i was younger, so i have a couple of tips:

a) if you masturbate (just a quick clean of the pipes) in advance your balls are way less sensitive

b) you might not have noticed but it gets triggered especially if anything (eg the girl, your underwear) touches against your balls so wearing baggy pants will make a huge difference.

Ya, but what if it gets so bad I cant even stand and want to throw up. You can't always just go and jack off every time a girl kisses you. I have noticed that it helps but is there a way to prevent it from hurting to begin with?

i gotta say im so relieved to find this post. ive been looking online for a bit now and i figured i must have Epididymitis or something, but the pain is gone the day after, so it didnt really fit...

so you guys just excuse yourselfes to the bathroom in the middle of a date and take care of yourselfes? i guess its a good idea if that hepls blueball-wise, but still kinda weird, no?

i used to get this a lot too so i tried not masturbating for a while and it pretty much stopped happening then the occasional jerk was fine and it never happened again
