Likewise, sometimes when I wake it due to other things but I take the time and urinate. For reassurance I use a shield when I am at the gym (5 days a week).

There are alternative treatments that do not interfere with sexual function but they do not give instant results.

I go to the gym three times a week and don't wear a pad for that. I don't have a problem. Of course they have bathrooms there which I use, but so do the young folks working out!

Me unlucky. My HoLEP seems to have failed. At week 4, my flow was within spec Qmax=21 cc/sec. But from week 5 to week 9(now), need to contract the bladder real hard to urine, flow became very slow, also urgency, long urination time, came back, finally penis slight pain during urination.

My plan is to do Qmax and retention locally, then show these results to dr das and request Dr. Das to do cystoscopy to check if there is new blockage formed after holep.

Definitely something wrong DL. Maybe scar tissue or something but yeah you need to get this checked out. Good luck, please keep us posted.

From what I've seen, it appears that some of those that had HoLEP procedures done where some tissue was left to help address incontinence seem to have flow and nocturia issues unlike me, who still have light incontinence after 13.5 months (still have to wear a shield daily for light leaks) but I flow like a racehorse and go all night without having to urinate. Even though the slowly improving but continued incontinence is frustrating, I count myself lucky to not be having some of the flow and nocturia issues I see some others having. I thought ur HoLEP was done by Dr.Das as well. If yes, I would think that he only removed 40% of the internal sphincter, just like in my HoLEP.

Yes he did mine DL, but we never discussed that aspect going in, and knowing the difficulty they had due to the unusual length of my prostate and the resulting long surgical time my procedure had, I believe my internal sphincter was pretty much destroyed, hence the continued incontinence issues I still have. I am seeing very slow improvement, so I still hold out hope for full recovery of continence, but it is indeed very slow going.

Thanks, Blayneb. I am sure u will eventually fully recovered. Buster’s prostate was over 200 cc way bigger than yours, and he is nearly fully recovered from his incontinence.

Yes I am hopeful too DL, thanks but it was the length of my prostate making it difficult to use their instruments that had Dr Das very concerned vs the overall size. In the end they were successful, but he did tell me they had to go slow and had a good deal of difficulty.

I had good foresight to pick Dr. Das who was able to operate successfully on the long prostate.

Indeed! He had warned me he may have to go the traditional prostatectomy route (which I had to agree to if necessary ahead of time) had they not been able to, but in the end they got it done thanks to his experience and skill so yes, I was fortunate to have him!

So u had cystoscopy by him before HoLEP? The long prostate was discovered during cystoscopy ?

No, he saw ultrasound results (plus measurements my uro provided in his notes) I had brought him from my local urologist during our pre-surgery consultation. He of course said he would confirm further for himself based on what he found when he went in during surgery but based on what he was seeing on that ultrasound he was concerned.

I am very surprised that he would let u have holep without first doing cystoscopy. Ultrasound could only show the shape but not was happening inside the prostate.

My local urologist had previously done a cystocospy DL (under anesthesia) and biopsy along with that ultrasound just a few months before I saw Dr Das and all those results along with his notes were provided to Dr Das so Dr Das saw no reason (as he said) to "put me though anything more" so we went right to surgery as soon as I was ready which I agreed to right away.
